The risk and severity of TD associated with conventional antipsychotic use can be reduced through a methodical approach that begins with a switch to an atypical antipsychotic other than clozapine. Other management considerations include, as necessary and in order, 1 ; discontinuation of anticholinergic therapy, 2 ; a switch to clozapine, 3 ; initiation of suppressive therapy with a conventional antipsychotic agent or with tetrabenazine, and 4 ; addition of one of the more experimental treatments, including donepezil, melatonin, branched chain amino acids, vitamin E or vitamin B6, and drug reduction see Figure 1 ; . Switching From Conventional to Atypical Antipsychotics Patients in whom TD develops after undergoing conventional antipsychotic therapy should be given an atypical antipsychotic and switched from the typical to the atypical gradually or abruptly, depending on both the patient and the medication characteristics 5962 ; . An atypical antipsychotic agent is usually selected on the basis of a patient's clinical profile and in consideration of the agent's efficacy data and specific adverse-effect profile. By birth, visual pathway is complete and images for different objects could be formed and perceived at visual center. Immediately after birth, infants see and look around. However, they still couldn't recognize or understand the images. Also development of eye muscles is complete and eye movements should be intact. During the first few months, shifting sight gaze towards light is elicited and babies start to recognize the mother and or familiar persons by seeing as revealed by the facial expression. At the age of 3 months the infant starts to play with his her hands. By 6 months, fixation reflex is intact and infant could fix sight gaze upon definite objects. During the first few months, infants become skilled users of their vision and develop eye-hand coordination. Both eyes normally look at the same object. The infant could identify certain objects by seeing, fixate on them and move the hand to catch them. During the first few years as the axial length for the eye increases the lens flatten. Myopic errors increase while hypermetropic errors decrease with age. It is important to diagnose a lazy eye early and to teach the infant to look with it. Otherwise that eye will be amblyopic and never see well.

MPTP evokes sustained activation of calpains in the SNc We first determined whether calpains may be activated in SNc neurons after chronic MPTP treatment in mice. Using an antispectrin body that selectively recognizes an epitope within that has been exposed by calpain proteolysis neoepitope ; Roberts-Lewis et al., 1994 ; , midbrain sections from mice treated with MPTP were analyzed for evidence of increased calpainmediated proteolysis by immunohistochemistry. Mice that were treated with saline did not exhibit any detectable increase in neoepitope labeling Fig. 2a ; , whereas a noticeable and sustained increase in calpain-mediated proteolysis was observed in the nigral region of mice that had been treated with MPTP Fig. 2b, c ; . Closer examination of this region revealed that the pattern of spectrin was located predominantly in the calpain-cleaved cytoplasm and was punctate in appearance Fig. 2e, f ; . This punctate appearance resembled calpain translocation to intracellular membrane compartments in hippocampal neurons after ischemia in vivo Yamashima et al., 1998 ; . Intracerebroventricular infusion of the pharmacological calpain inhibitor MDL-28170 prevented the emergence of immunoreactivity for calpaincleaved -spectrin after chronic MPTP administration Fig. 2d ; . To demonstrate that the observed increase in calpain-related activity indeed was in nigral dopamine neurons, we performed immunofluorescent double-labeling experiments using TH as the dopaminergic marker.
